Even the most sophisticated risk management system is only as strong as its ability to identify risk and treat it before it has any impact. This requires relevant, reliable, and objective inputs and a dynamic system that accurately forecasts the likelihood and impact of risk. Yet, too many risk management systems fail to do so.

Too many companies are caught completely off guard by risk events. Why?   

For starters, even good teams are prone to confirmation biases, ignore early warning signs, and misdiagnose risk events. Many risk management systems are driven by risk registers that use flawed risk methodologies, are based on subjective and biased inputs, and rarely get updated because only a few individuals have access. In this way, many risk management systems are static and fail to identify impending risk events.
